Our attempts to unite behavior therapeutic with neuropsychological approaches to rehabilitation were largely instigated by our observation that brain-damaged patients frequently did not have the same level of desire for restoration of function that is commonly seen in patients with physical disabilities. The sources of motivational lag are generally unclear. They may be secondary to the patient's intellectual deficits, they may be directly related to structural or physiological alterations of the brain mechanisms underlying motivation, or they may be related to the same phenomena that reduce motivation in nonbrain-damaged individuals, such as "learned helplessness" related matters.

We mean something more specific here than the somewhat vaguely defined relationship between the desire to get better and the course of recovery.
